A non-empty username and/or a non-empty were provided. Not deauthenticating properly can result in a security risk allowing someone else to use the username/password stored in the browser to later access this application.

Retry but make sure to clear, if necessary,  the username and password fields in the dialog that will be displayed by the browser.
